Classic/Traditional vs Modified/Mainstream (4:1 vs 2:1)

The classic keto diet consists of a ratio in grams of fat to nonfat (protein and carbs) of 4:1 and 3:1 – now popular keto diet is slightly modified and consists of ratios of 2:1 and 1:1
* 90% fats, 10% protein, less than 1% carbs*

The problem with this diet for anything other than medical use is obvious: protein intake is very low, which isn’t optimal as proteins are the primary building blocks of the body. They’re used to build tissues like muscle, tendon, organ, and skin, as well as many other molecules vital to life such as hormones, enzymes, and various brain chemicals. It has been found that a ketogenic state can still be maintained with an increase in protein and a slight bump in carbs. The type of ketogenic diets popular today are generally high-protein variations of the original – Protein recommendations can vary, ranging from 20 – 30% of daily calories, but carb intake is set to 50 grams or less per day, with the rest of your calories coming from dietary fat.

Ketosis and Disease Treatment:
The ketogenic diet is well established as therapy for many disease states and should be considered first-line therapy in glucose transporter type 1 and pyruvate dehydrogenase deficiency. The ketogenic diet appears effective in other metabolic conditions, including phosphofructokinase deficiency and glycogenosis type V (McArdle disease). It appears to function in these disorders by providing an alternative fuel source. A growing body of literature suggests the ketogenic diet may be beneficial in certain neurodegenerative diseases, including Alzheimer’s disease, Parkinson’s disease, and amyotrophic lateral sclerosis. In these disorders, the ketogenic diet appears to be neuroprotective, promoting enhanced mitochondrial function and rescuing adenosine triphosphate production. Dietary therapy is a promising intervention for cancer, given that it may target the relative inefficiency of tumors in using ketone bodies as an alternative fuel source.

Epilepsy Study:
A study published in the journal The Lancet Neurology, in 2008, showed the efficacy of the ketogenic diet in children. In 145 children, aged 2 to 16 years old, who had at least daily seizures and had failed to respond to at least 2 antiepileptic drugs for 3 months, the mean percentage of baseline seizures was significantly lower in the diet group than in the control group (62% vs 137%, 75% decrease) Twenty-eight children (38%) in the diet group had greater than 50% seizure reductions compared with 4 (6%) controls, and 5 children (7%) in the diet group had greater than 90% seizure reductions compared with no controls.

  1. Atkins Original Diet works. Keto adds fruit etc. and is healthy but not nearly as effective for weight loss.

  2. The Atkins diet has changed since the death of Robert Atkins in 2003, and I suppose that this is where "modified Atkins" comes into play. However, the original Atkins (1972) and New Diet Revolution (1992) both had an initial period that were very much ketogenic. This period was initially called "Level 1" and later termed "induction". Robert Atkins did not advocate counting Protein vs Fat macros as long as carbohydrates were liminated to 20g, but if you look at the meal plans in these books, many are 2:1 and a few are very close to 1:1. In the first book, Dr. Atkins suggested this way of eating for the first week, but in "New Diet Revolution", it was extended to at least two weeks with the option to keep it going as long as the dieter wanted.

    Chapter 18 of "New Diet Revolutions" titled "Treating Extreme Metabolic Resistance" has a plan called a "fat fast" that limits calories to 1000 per day, with 90% as fat. So this Atkins plan would certainly create a state of ketosis.

  4. I’m reading the original Atkin’s diet book from 1972. Keystones and ketosis are discussed quite a bit. It really is like the Keto diet. The New Atkins is exactly as you describe. There is a definite difference! Always love your videos.

  5. Before trying the *Agoge diet*, I tried the keto diet and many other diets but none worked. With the *Agoge diet*, I lost so much stubborn belly fat, finally getting that lean look.

  7. I find most of these diets tough to adhere to, including the ketogenic diet. The thing that worked best for me is the Agoge Diet plan. You get to eat nutritionally rich foods without starving.

  8. Keto has been shown to be risky in numerous studies, especially without doctor supervision. The Agoge diet was the only diet that helped me lose that stubborn fat without abuseing my body.

  10. The original Atkins has no ratios. The original book written in the 70's (not this new website that came about after dr. Atkins death) states that Atkins is a fat based diet. I'm not sure who modified the diet but it wasn't dr. Atkins. I'm starting induction again after the lockdowns in California. Atkins is simple and very effective and you know you're in ketosis when you go to sleep wake up and lose a lb or 2. I lost a significant amount of weight on Atkins and it was supper simple.

  11. please do more videos like this Tom… I am interested in the theraputic side and the foods/fats/calories to have per day that will support that level of theraputic ketosis and keeping your weight down without loosing too much muscle in the process.. Videos diving into what might happen if you go into theraputic ketosis and what will happen to your muscle/ body composition without a lot of protein (even though I know ketones are muscle sparing) I even had a dream about you the other week which tells me how much I love your channel and style of teaching. love from London xx

  12. This is an old video so don't know if anyone will ever see this comment, but whats the recommendation in achieving a 3:1 or 4:1 fat to protein ratio? Is it to maintain protein levels at a .8-1g/lb of body weight, but increase your fat intake so its 3 to 4 times that? Or is it to lower protein intake below the .8g/lb of bodyweight threshold?

  13. This makes perfect sense as to why I overeat protein and ketones are gone. It's not terrible since I'm more in maintenance than anything. But I absolutely have to moderate protein to maintain ketosis. It's also nice knowing I don't need carbs in order to break ketosis so I don't have to go out of my way with carb rich foods to do so. Maybe because I'm not fat adapted (actual ketosis) yet is why this happens… but it does happen

Fasting and exercise are beneficial and may speed up the weightloss but if you have to fast or exercise to get results on ketogenic diet then you're probably consuming too much caffeine and / or the wrong sweeteners or you're sensitive to even ketogenic sweeteners. Or all three are your issue. Eating fat , especially the rich animal fats, causes the body to ketose more heavily. Caffeine spikes insulin so it can slow weight loss to the point of stalling. Dr. Atkins explained all this in his original New Diet Revolution book. When I started ketogenic dieting again I was too sensitive to even ketogenic sweeteners so I had to lay off of them to speed up weight loss. And caffeine definitely slowed my ketosis too much so I ditched it completely. From experience I don't trust all decaf anymore either. But some did do well with me.

  17. I was a healthy man and thought I could improve my performance even more with a keto Atkins diet. I gained weight, got horrifically negative mood, got sky high cholesterol, developed heart arrythmia, got clogged arteries and finally a disabling stroke. May Atkins rot in hell for destroying my health!!! The world is a better place without that devil and charlatan. I cannot express my hate enough

  19. I do IF and Atkins. Mostly because I don't like to be too numerical with my food. Easier for me to stick to the diet.

  20. The question is, does keto or Atkins kill you faster? I would say that it depends, but both will screw up your arterial health and you will get some form of severe cardiovascular complications. So you choose.
